RE: DISCLOSURE REPORTING PAGE TO FORM U-4

As described in the attached NASD Notice to Members Number
88-97, Form U-4 has been revised to include a Disclosure
Reporting Page (DRP). Its implementation is effective
immediately.
The DRP consists of a one-page attachment to Form U-4,
developed for use by NASD-member firms to report in detail the
disciplinary history of their registered representatives. An
affirmative answer to Item 22 of Form U-4 concerning past
disciplinary problems must be explained on the DRP. A separate
DRP must be submitted to the CRD System for each disciplinary
event being disclosed. Attached is a copy of the DRP and a
sample DRP for your review.
For Investment Advisers Guide recipients, attached is a
complete Form U-4 including the DRP and an attachment sheet
revised to include specific reference to Items 18, 19 and 20 of
Form U-4. We suggest you insert the entire form in the Forms
section of your Guide, behind Form ADV.
