MEMBERSHIP APPLICATION QUESTIONNAIRE FOR INFORMATION REPORTING PROGRAM ADVISORY COMMITTEE

May 3, 1993 TO: TAX COMMITTEE NO. 24-93 OPERATIONS COMMITTEE NO. 18-93 TRANSFER AGENT ADVISORY COMMITTEE NO. 31-93 RE: MEMBERSHIP APPLICATION QUESTIONNAIRE FOR INFORMATION REPORTING PROGRAM ADVISORY COMMITTEE __________________________________________________________ As we previously informed you, in 1991 the Internal Revenue Service ("IRS") established the Information Reporting Program Advisory Committee ("IRPAC") and I was appointed to the Committee. (See Institute Memorandum to Tax Members No. 47-91, Operations Members No. 27-91 and Transfer Agent Advisory Committee No. 50-91, dated November 5, 1991.) Since its creation, IRPAC has been very active in providing IRS with the views of the payor community on a variety of information reporting issues, including issues such as information reporting to corporations, backup withholding and cost basis reporting. Appointments to IRPAC are for two-year terms. The Committee holds formal meetings twice a year, its subcommittees hold frequent meetings and its members are in constant contact with IRS. In the attached notice, the IRS announces that all persons interested in membership on the Committee for 1994 and 1995 should complete the attached questionnaire of 23 items and send it to IRS no later than May 29, 1993.
