RE: REVISED FORM W-8 (CERTIFICATE OF FOREIGN STATUS)

The IRS has issued a revised version of Form W-8,
Certificate of Foreign Status. Both the IRS announcment of this
revision and the revised form are attached.
The February 1991 revision eliminates the request for visa
information, reinstates the provision for a permanent address and
revises the language of the certification of foreign status.
Any 1989 Form W-8 or substantially similar statement
completed and signed before January 1, 1992 will remain valid.
After the date, the February 1991 version or substitute statement
must be used.
