RE: FREE SOFTWARE FOR COMPUTER GENERATION OF FORM 5307

Attached is IRS Announcement IR 90-78 regarding computer
generation of Form 5307, Application for Determination for
Adopters of Master or Prototype, Regional Prototype or Volume
Submitter Plans and Form 8717, User Fee for Employee Plan
Determination Letter Request. The IRS will provide software,
free of charge, to sponsors of prototype pension plans to create
a computer generated application in lieu of using pre-printed
Form 5307 (see Institute Memorandum to Pension Members No. 19-90,
dated April 24, 1990) and Form 8717.
To obtain the software, plan sponsors must submit a 5 1/4"
formatted diskette to the key district office in which they are
located, at the addresses listed in the attached notice.
