RE: IRS ISSUES DRAFT COPIES OF REVISED FORMS 5300, 5302, 5307
AND 6406

Attached is a copy of IRS Announcement 90-53, which
includes draft copies of revised Forms 5300 (Application for
Determination for Employee Benefit Plan), which will now cover
both defined benefit and defined contribution plans, 5302
(Employee Census), 5307 (Application for Determination for
Adopters of Master or Prototype, Regional Prototype or Volume
Submitter Plans), and 6406 (Short Form Application for
Determination for Amendment of Employee Benefit Plan).
The forms have been revised to reflect changes made by the
Tax Reform Act of 1986 and generally will be available for use by
April 23.
