of Employee Benefit Plan (With fewer than 100 participants),
along with the instructions thereto.
Previous Forms 5500-C and 5500-R have been combined into
the attached form for 1989. If Form 5500-C otherwise would have
been filed for 1989, the plan sponsor would complete pages 1 and
3 through 6 of Form 5500-C/R. Form 5500-R filers would complete
only pages 1 and 2 of the new form.
