[18258] November 30, 2004 TO: CHIEF COMPLIANCE OFFICER COMMITTEE No. 30-04 RE: SURVEY OF COMMITTEE MEMBERS REGARDING CCO COMMITTEE MEETINGS - RESPONSE REQUESTED BY DECEMBER 6TH At the November 16th CCO Committee meeting, we distributed a brief survey questionnaire seeking input from Committee members to help us maximize the value of CCO Committee meetings. The survey addresses topics such as members’ reasons for attending the meetings, the length and frequency of meetings, the usefulness of the meeting materials, etc. Committee members were asked to complete the survey at the meeting. We would like the survey results to reflect the responses of as many Committee members as possible. Therefore, if you (or an alternate) did not attend the November 16th meeting, please complete the attached survey and return it to us by Monday, December 6th.
